Instruction Sheet for Undergraduate Application
|
The following instructions correspond to the numbers on the application:
1.Name-print your full legal name as it appears on your passport. Please use this name on all correspondence.
2.Permanent Home Address-an address that stays the same as you move around the world. Most often this will be your parent's address.
3.Mailing Address-The address where you are receiving your mail at the present time if different from your permanent home address. The telephone number and fax number should include country and city codes.
4.If you have an e-mail address or a Social Security Number please fill in the blanks. If not, mark the blanks N/A (not applicable).
5.Please print or type (City, Province or State and Country) where you were born. Please list your date of birth as month/day/year (i.e. January 1, 19--)
6.You are a citizen of the country who issued your passport. Gender: be sure to check the appropriate box.
7.Please list your date of birth as month/day/year (i.e. January 1, 19--). Please type or print this information.
8.Please check the box that fits best in the Ethnic Group.
9.Please check the appropriate box and print or type name of your parent, legal guardian or other individual who should be notified in case of an emergency.
10.If you need to check on the majors offered, a list of majors is available through the resources listed.
11.Please fill in the year you wish to start at SDSU. You will need to be here in August for the Fall semester and December for the Spring semester.
12.Fill in appropriate box. If answered yes, nothing need be noted for native language.
13.A 500 minimum TOEFL (paper based score or the equivalent) is required for admission. The score should be no more than two years old. The ACT or SAT is not required for international students. However, if you have taken either of these the scores should be sent to our office.
14.Check the correct response and fill in graduation date or anticipated graduation date.
15.If you have applied before, we may still have the documents you submitted on file. The date will help us locate existing documents. This information is needed to combine old and new files together.
16.This information is important. It helps us understand your current status.
17.Fill in all schools attended starting with grade 9 and continuing to the present.
18.An original transcript (or certified copy) of your secondary and post-secondary school marks or grades in each subject for each year is required. Include your rank in class on the transcript if possible or in a separate letter from a school official. Also send certified copies of any certificates, diplomas or degrees awarded with your application.
INCLUDE A SYLLABUS OR DETAILED DESCRIPTION OF ALL COLLEGE LEVEL COURSE WORK TAKEN OUTSIDE THE U.S. TO ASSIST IN DETERMINING YOUR ELIGIBILITY FOR ADMISSION AND FOR TRANSFER OF CREDIT.
19.List all activities when not attending secondary or post secondary institutions including work, voluntary activities etc. until your entrance to SDSU.
