Federal PLUS Request Form

A parent must complete this form to request the amount of PLUS for the academic period. In addition, the parent must complete or have previously completed a PLUS Master Promissory Note (MPN) with the SDSU Financial Aid Office. If a MPN is required, the lender's guarantee agency will send the parent information on completing the MPN to the parent.

I request the school certify a PLUS for the following amounts with the lender listed below: (Students must be enrolled in a minimum of six credits from one of the six state universities in the SD system for the loan period) CHOOSE ONLY ONE:

I approve my PLUS to be paid by Electronic Funds Transfer (EFT) on my dependent’s SDSU bill and, if applicable, refund any credit balance to my son or daughter. If EFT is not approved or the lender you choose does not use EFT, a PLUS check payable to the parent and SDSU is sent to the SDSU financial aid office. The check is then endorsed by SDSU and sent to the parent, this extra step will cause a delay in payment processing.
